2010-2014: Community Health Improvement Plans from Minnesota CHBs - Minnesota Dept. of Health

2010-2014: Community Health Improvement Plans from Minnesota Community Health Boards

A community health improvement plan is a long-term, systematic plan to address issues identified in the community health assessment.

The purpose of the community health improvement plan is to describe how the health department and the community it serves will work together to improve the health of its population.

In Spring 2015, Minnesota's community health boards submitted their community health improvement plans to the Minnesota Department of Health as part of Minnesota's Local Public Health Assessment and Planning process.

Please Note: Community health boards took multiple approaches to incorporating health equity and social determinants of health in their community health improvement plans. Some identified it as a specific community health priority, which is reflected on the list above. Others included it in another section of their plan or wove it throughout the document. If health equity/social and economic factors are not identified above as a theme for a particular community health board, their plan may prioritize health equity or social and economic factors in other ways.
