2010-2014: Community Health Improvement Plans from Minnesota Community Health Boards
A community health improvement plan is a long-term, systematic plan to address issues identified in the community health assessment.
The purpose of the community health improvement plan is to describe how the health department and the community it serves will work together to improve the health of its population.
In Spring 2015, Minnesota's community health boards submitted their community health improvement plans to the Minnesota Department of Health as part of Minnesota's Local Public Health Assessment and Planning process.
