2010-2014: Quality Improvement Plans

A quality improvement (QI) plan is an internal document that describes how the organization will manage, deploy, and review quality throughout the organization. It discusses how quality improvement approaches, methods, tools, and techniques, as well as application of lessons learned from evaluation will be used throughout the organization.

In Spring 2015, Minnesota's 48 community health boards submitted their QI plans to the Minnesota Department of Health as part of Minnesota's Local Public Health Assessment and Planning process. As QI plans are internal organizational documents, they are not posted on this website. If you are interested in obtaining your community health board's QI plan, please contact your community health board directly.

Community health boards are not required to submit QI plans as a deliverable for the 2015-2019 Assessment and Planning cycle.
