Rethinking Rural Health Care: A Community Effort

Each year the Minnesota Rural Health Conference honors one outstanding individual and one team in the field of rural health. This year’s nominations were due May 6, 2009. The awards will be presented at the Conference Awards Luncheon on June 16.

Award Categories: Rural Health Hero and Rural Health Team

The Minnesota Rural Health Hero of the Year and The Minnesota Rural Health Team of the Year demonstrate what it means to go “above and beyond” what is required to provide quality health care. The awards committee will consider outstanding care, involvement in the community, and lasting contributions to the health care system.

Selection is based on how the candidate's compassion, innovation, leadership, collaboration, efficiency or unselfishness contributes to improved rural health care—whether the effort is volunteer or paid.

Areas of impact could include, but are not limited to, health care and health programs, rural health policy, legislation, the advancement of the field, improved patterns of health care delivery, cooperative efforts to avoid duplication of services and achieve common goals, and the lasting impact of a program on populations and areas served. Include the significance of the work to the community, populations or individuals served.

Nomination Materials
Complete the nomination form ( Word: 45KB/1pg or PDF: 14KB/1pg )
Write the narrative: This is the primary instrument used to judge the nomination. Describe, in up to three double-spaced pages using a 12-point font, why you believe the person or group being nominated should be considered based on the criteria listed above.
Additional Attachments
You may submit up to four pages of relevant news clippings or supporting letters. For example, two 8 1/2 x 11 pages of news articles and two one-page letters of support constitute the limit of four pages, or three letters and one article, or four letters and no articles—any combination up to four is acceptable.

Resubmission of Nominations
Individuals or teams nominated in the past—but not selected—may be resubmitted.

Submission of Nomination
Nomination forms, narratives and attachments must be e-mailed to Only Microsoft Word, Excel, Powerpoint, PDF, or text files can be accepted. All nomination materials were due May 6, 2009.

Selection Process
All award determinations will be based on the merits of nominees, relevance to established criteria and demonstrated benefit to rural areas served. Programs of regional, state or national scope are given equal consideration in the nomination process. Winners are determined by a committee of Minnesota Rural Health Conference partners.

Awards Presentation and Recognition
The 2009 Rural Health Awards will be presented during a luncheon ceremony at the Minnesota Rural Health Conference on June 16.
