Successful Model for Rural and Underserved Communities

The Consortium at Carondelet Village

Project: Collaborative group of health and wellness providers helping area residents live vibrant and independent lives in their homes.

Geographic Area: Older adults living in the St. Paul neighborhoods of Highland Park, Mac-Groveland, West Seventh, Summit Hill, Summit University and Union Park.

Description: The Consortium at Carondelet Village is a personally-designed care management approach that recognizes and builds on each individual's unique strengths (physical, spiritual, emotional, intellectual, vocational and social). The care manager works with each person to identify individual strengths and desires for engagement. We promote independence, community engagement and coordinate services that help manage chronic and complex conditions through evidence based assessments care management, chronic disease management, transitional care, and health promotion models.

We inspire older adults to identify strengths and connect with the broader community through a variety of wellness, health promotion, and volunteer resources, which include:

  • Educational programs
  • Spiritual enrichment
  • Coordinated home health care
  • Meaningful volunteer opportunities
  • Health systems navigation
  • Community involvement
  • Assessments and support
  • Transportation services
  • Information and referrals
  • Dining options.

This initiative is being funded through June 2012 by a three-year grant from the Minnesota Department of Human Services. Carondelet Village is a partnership of the Sisters of St. Joseph of Carondelet, St. Paul Province and Presbyterian Homes.

Plans for the Future
Carondelet Village, a senior residential continuum of care campus, will open in late 2011 on the corner of Randolph and Fairview Avenues in St. Paul at the campus of the Sisters of St. Joseph, adjacent to St. Catherine University. Within the heart of Carondelet Village, Carondelet Circle will become the home of The Consortium at Carondelet Village to provide seniors with wellness, health and holistic programs and services in a community setting so they may live well wherever they call home.
