STATE HEALTH REFORM Health Care Home team hosts conference calls and webinars every third Wednesday. There will be a brief update on health care homes implementation and next steps, followed by a brief presentation on a certification-related theme. Each month focuses on a different theme. Questions are welcome.
HEALTH INFORMATION TECHNOLOGY NEWS AND RESOURCES Community Clinic Grants were awarded to C.A.R.E. Clinic (Red Wing), Community Dental Care (Maplewood), Community University Health Care Clinic (Minneapolis), Face to Face Health and Counseling (St. Paul), Family Medical Clinic (Minneapolis), HealthFinders Collaborative (Northfield), Lutheran Social Services Wellness Center (Duluth), North Metro Pediatrics (Coon Rapids), Open Cities Health Center (St. Paul), Scenic Rivers Health Services (Cook), Southside Community Health Services (Minneapolis), St. Mary’s Health Clinics (St. Paul) and West Side Community Health Services (St. Paul). The Community Clinic Grant program helps clinics serve rural and urban populations with low incomes, reduce current or future uncompensated care burdens, or provide improved care delivery infrastructure. STAFF NEWS Judy Bergh visited Chippewa County Hospital (Montevideo), Granite Falls Municipal Hospital and Manor and Renville County Hospital. Anne Schloegel and Amy Vallery joined Judy to tour Mille Lacs Health System in Onamia. Jill Myers and Kristen Tharaldson traveled to Duluth to meet with staff at the Center for Rural Mental Health Studies, and Jeff Plunkett at SISU, and Pam Bjorklund at College of St. Scholastica on behalf of the Rural Health Advisory Committee's Rural Telemental Health Work Group. Mark Schoenbaum attended the open house Southside Community Health Services held celebrating its newly remodeled administrative offices and dental clinic. Kristen Tharaldson was in Kanabec County to meet with Wendy Thompson, director of health and human services, and Chuck Hurd, director of adult services/telemental health coordinator, about telemental health services being provided through an "extender model" in four counties in central Minnesota. | |||||||||||

Universal Medical Services (AXIS Medical Center) was designated as a Federally Qualified Health Center Look-Alike, effective April 1. AXIS provides comprehensive health services to the medically underserved in South Minneapolis. The center is serving more than 3,000 people; 93 percent are Medicaid recipients. ORHPC's Craig Baarson assisted AXIS in obtaining FQHC designation. Lakewood Health System developed a Medical Home Training Package. The cost is negotiable depending upon quantities and distribution through state and regional nonprofit organizations. Contact Kristen Svobodny at 218-894-8577. The University Partnership for Health Informatics (UP-HI), a collaboration of University of Minnesota's Institute for Health Informatics, The College of St. Scholastica School of Nursing and School of Health Sciences and the University of Minnesota-Crookston, received $5.1 million in federal stimulus funding to train health IT professionals. Fremont Community Clinic celebrated its 40-year anniversary at Target Field. | ||
