Procedure for receiving a J-1 visa waiver application case number
To start the J–1 visa waiver application process, an applicant needs to communicate with both federal and state agencies. Please carefully read the following instructions before applying for a case number.
The Minnesota Department of Health (MDH) recommends that before submitting any paperwork, applicants should contact Jodi Millner at 651–201–3856 or email@example.com to determine that the employer/facility and the applicant/physician both meet the minimum requirements to qualify for a waiver. The determination that the employer and the applicant meet the minimum requirements does not guarantee that the full application will be approved by MDH.
