Procedure for Receiving a J-1 Visa Waiver Application Case Number

To start the J-1 visa waiver application process, an applicant needs to communicate with both federal and state agencies. Please carefully read the following instructions before applying for a case number.

The Minnesota Department of Health (MDH) recommends that before submitting a Data Sheet and filing fee, applicants contact Lawrence Colaizy at (651) 201-3851 or Lorry Colaizy to determine that the employer/facility and the applicant/physician both meet the minimum requirements to qualify for a waiver. The determination that the employer and the applicant meet the minimum requirements does not guarantee that the full application will be approved by MDH.

US Department of State Requirements:

Prior to applying for a J-1 visa waiver an applicant must send a completed Data Sheet, two self-addressed, stamped, legal-size envelopes (S.A.S.E.), and a filing fee to the United States Department of State and receive a case number. The case number must appear on all documents filed with the application.

Minnesota Department of Health Requirements:
