Connecting Community Data for Population Health
Public Health 3.0 (PH3.0) calls for local public health to lead the way in leveraging cross-sector data to address community health needs, especially around social determinants of health. In Minnesota, many communities have been meeting this call to action by collaborating across clinical care and local public health to mobilize data to knowledge and ultimately improve the health of all people in their communities.
