Health care homes

  • The Minnesota Department of Health has certified 44 new health care homes. This brings the total number of certified health care homes across the state to 91, with many more in the process of becoming certified.

Weekly Update - February 10, 2011

Statewide quality reporting - Quality Incentive Payment System

The Minnesota Department of Health (MDH) is preparing its first proposed update to the Minnesota Quality Incentive Payment System since the initial framework was published in January 2010. The updated framework will be available on Monday, February 14, 2011, at Health Care Quality Measures - Quality Incentive Payment System.

Interested stakeholders may submit comments on the updated framework to health.reform@state.mn.us through February 28, 2011.


Provider peer grouping

MDH's February 14 conference call about the provider peer grouping initiative has been canceled. The monthly conference calls will resume in March.


Health care homes

  • The Minnesota Departments of Health and Human Services have selected the Institute for Clinical Systems Improvement (ICSI) to lead a statewide health care home learning collaborative. The goal of the learning collaborative is to build the capacity of primary care providers to meet the standards, criteria and expected outcomes of health care home certification and to meet state mandates for health care homes enacted during the 2008 legislative session (M.S 256B.0751).

    Under this contract ICSI will:
    • Convene a health care homes learning collaborative leadership committee comprised of diverse health care community stakeholders to provide oversight for the project.
    • Develop an appropriate health care homes learning collaborative curriculum, with emphasis on helping certified clinics/clinicians.
    • Design, implement and coordinate the learning collaborative statewide.
    • Track the effectiveness of the learning collaborative activities through a comprehensive evaluation plan.
  • MDH has contracted with MN Community Measurement (MNCM) to collect health care homes quality data in 2011, co-chair the measurement technical work group and convene a work group for the development of a new health care homes/functional status measure. MDH and MNCM will communicate additional information about measurement activities in the near future.

  • The health care homes program wants to notify primary care practices about an electronic health record (EHR) loan program administered by MDH's Office of Rural Health and Primary Care. The loan program helps finance the implementation or support of interoperable EHR systems, including EHR software, hardware, training and support. Applications are due April 1.

    Loans are six-year, no-interest with the first year’s repayment deferred. Information on eligible entities and the application process is available online at Electronic Health Record Loan Program.

Statewide quality reporting

MN Community Measurement’s Measurement and Reporting Committee will meet February 9 on work related to the development of the statewide quality reporting and measurement system.
