Health care homes
- The Minnesota Department of Health (MDH) has announced the quality measures that will be used in 2011 during the first year of certification as a health care home. Starting in 2011, medical clinics and clinicians participating as certified health care homes will be expected to submit measurement data. This data will be used for benchmarking, recertification and overall evaluation of the health care homes program.
MDH is committed to aligning health care home quality measurement reporting with existing efforts in the state, including the Statewide Quality Reporting and Measurement System. More detail on the specific measurement tools and data submission process will be released when available.
The initial measures include:- Optimal vascular care
- Optimal asthma care
- Patient experience
- Cost measurement
