Statewide quality reporting

REMINDER: The Minnesota Department of Health invites interested stakeholders to submit recommendations on the addition, removal or modification of standardized quality measures to MDH by June 1, 2011. Additional information about the standardized quality measures and the criteria for recommendations is available at Health Care Quality Measures - Recommendations.

The deadline for submitting recommendations on the standardized quality measures is 4:30 p.m. on June 1, 2011. Please send your recommendations to health.reform@state.mn.us.

Health care homes

Information about the Department of Human Services' demonstration project to test alternative and innovative health care delivery systems is now available at www.dhs.state.mn.us/provider/HCDeliveryDemo. The Web page will be updated as the project progresses. Individuals also can sign up to receive email updates about the project. The department previously issued a request for information and is now reviewing the responses as part of developing a request for proposals to be issued in late spring or early summer.

Statewide quality reporting

REMINDER: The Minnesota Department of Health invites interested stakeholders to review and comment on MNCM’s preliminary recommendations for physician clinic and ambulatory surgical center measures for the 2011 administrative rule. The preliminary recommendations and additional information are now available at Health Care Quality Measures – Recommendations.

The deadline for comments is May 15, 2011. Please send comments to health.reform@state.mn.us.

Health care homes

  • REMINDER: The deadline for proposals for the Health Care Homes: Safety Net Clinic Primary Care Transformation Grant and the Health Care Homes: Community Care Team Grants is May 16, 2011.

  • REMINDER: The Minnesota Department of Health has issued an informal solicitation for a health care home purchaser/employer partnership project. This project involves outreach, education and dialogue between state government (Minnesota Department of Health and Minnesota Department of Humans Services) and self-insured purchasers of health care regarding the statewide multipayer health care home (HCH) initiative.

    The deadline for responses is May 18, 2011. More information is available at Health Reform Requests for Proposals.

  • The Minnesota Department of Health has certified 43 new clinics and additional departments in two clinics that were already certified. There are now 134 certified health care homes in Minnesota.

    The new certified clinics are:

Weekly Update - May 5, 2011

Provider peer grouping

MDH will hold monthly conference calls to share information about the provider peer grouping initiative on the second Monday of each month from 7:30-8:15 a.m. The next call-in session is scheduled for Monday, May 9. The presentation slides for the May 9 call will be available on Friday, May 6, on the Provider Peer Grouping page.

Statewide quality reporting

  • The Hospital Quality Reporting Steering Committee will meet Tuesday, May 10, from 2:30-4:30 p.m., on work related to the development of the Statewide Quality Reporting and Measurement System.

  • MN Community Measurement’s Measurement and Reporting Committee will meet Wednesday, May 11, on work related to the development of the statewide quality reporting and measurement system.

  • Registration is now open for the first phase of the Health Care Home Statewide Learning Collaborative, sponsored by the Minnesota Department of Health and the Institute for Clinical Systems Improvement.

    This preparatory phase is designed to help clinics and clinicians understand and plan for the requirements of health care home certification and to prepare for related culture change.

    Participants can choose from one of these three series:
    • May 25 - July 19, 2011
    • September 6 - November 4, 2011
    • November 7 - January 12, 2012
