Webinar: Organizational Tips to Improve Your Quality Measures - September 25
All medical groups want to report high quality care to Minnesota Community Measurement and the Statewide Quality Reporting and Measurement System. In this webinar Dr. Kottke will draw upon his regional, national and international experience to suggest actions that medical groups can undertake to achieve their health care homes performance goals. He will also discuss illustrative cases from the medical literature that can help the participant improve their own scores.

Upcoming Minnesota State Innovation Model (SIM) task force meetings
The SIM Multi-Payer Alignment Task Force and the SIM community Advisory Task Force held a joint meeting on August 29. The Minnesota Department of Human Services and the Minnesota Department of Health have appointed these task forces to support the Minnesota Accountable Health Model initiative, which aims to improve community health and ensure that every Minnesotan has access to team-based, coordinated, patient-centered care. Minnesota was awarded a $45 million State Innovation Model (SIM) grant from the Centers for Medicare & Medicaid Services in February 2013. The initiative's three-year testing period begins October 1.

Minnesota Department of Health announces two newly certified health care homes (HCH)
The newly certified clinics are:
- Sanford – Wheaton Clinic
- Sanford – Wahpeton Clinic

Patient experience webinar for providers and clinics: "Statewide Quality Reporting: Building the Bridge to Better Care through Patient Experience Data.", Aug. 28, 7:30-9 a.m.
On August 28, from 7:30 to 9 a.m., the Minnesota Medical Association will host a webinar about the CG-CAHPS Patient Experience of Care Survey data recently released on the MNHealthScores website. Presenters include the Minnesota Department of Health, Minnesota Community Measurement, Dale Shaller (a national CAHPS expert), and Allina Health. This webinar will help providers and clinical staff better understand the data, how it was calculated, and how to use it for quality improvement, and will provide perspective on the survey's importance in the context of state and national health reform. Health Care Homes Learning Collaborative hosting webinar: "Making Connections: Introducing a Toolkit to Assist in Working with Patients with Complex Needs", Aug. 29, 12:30-1:30 P.M.
This webinar is intended to orient the audience to a toolkit that has been developed to assist health care homes and primary care clinics in serving patients with complex needs. The goal of the toolkit is to offer an array of resources, tools, and guides for building a model of care that effectively services seniors and persons with complex functional support needs in addition to their medical issues. The toolkit was developed as a part of Minnesota's Medicare Multipayer Advanced Primary Care Practice (MAPCP) Demonstration Project. Hypertension Management Workshop offered, Sept. 17 in Willmar, Sept. 18 in Marshall
Health Care Homes, Community Transformation Grant, and Heart Disease and Stroke Prevention Unit present, "Teamwork & Skill-Building to Transform Hypertension Management: Screen, Counsel, Refer, and Follow-up." The workshop is being offered at no charge for participants on September 17 in Willmar and on September 18 in Marshall.
Content will focus on management of hypertension within the overarching framework of team-based, coordinated, patient-centered care, and the integration of primary care and community resources.

Minnesota conducts nation's largest statewide patient experience survey
MDH and MN Community Measurement released patient experience data for 651 clinics Wednesday. The data is collected as part of Minnesota's Statewide Quality Reporting and Measurement System and is the most comprehensive look at patient experience on a statewide basis in the country.
Health Care Homes meeting
The Health Care Homes (HCH) Learning Collaborative Advisory Committee will meet August 22, 2013 from 1-3 p.m. at the Dakota County Northern Service Center, 1 Mendota Road West, West St. Paul. The primary purpose of this group is to provide guidance and technical expertise to the HCH Learning Collaborative. This committee works closely with the HCH Staff to plan the activities, evaluation, and curriculum for the Learning Collaborative.

Statewide Quality Reporting and Measurement System requests comments
On August 12, MDH published proposed amendments to Minnesota Rules Chapter 4654 (the Minnesota Statewide Quality Reporting and Measurement System) in the State Register. MDH posted the proposed changes, including changes to the rule's appendices, at Health Care Quality Measures – Proposed Rule Amendment. The publication of the proposed amendments initiated a formal 30-day comment period. Comments must be submitted by 4:30 p.m. on September 11, 2013. MDH is required to annually review and publish any proposed amendments to Minnesota Rules Chapter 4654 by August 15 of each year.

Learning Collaborative Evaluation Proposal resleased by MDH, Health Care Homes
Minnesota Department of Health, Health Care Homes invites proposals to design an evaluation plan and electronic tools to evaluate the effectiveness of the HCH Statewide Learning Collaborative.
Responses will be accepted until August 26 at 4 p.m.. Work is expected to begin in early September to be completed by September 30.

MNsure seeks for advisory committee members
The MNsure Board is establishing two Advisory Committees to provide guidance, advice and recommendations to the Board as it carries out its mission. Applicants are being sought for the Health Industry Advisory Committee and the Consumer and Small Employer Advisory Committee. More information and the application form are posted here.
Applications are due by 3 p.m. on Thursday, August 15, 2013.
MNsure seeks comments on proposed rules
MNsure is soliciting comments on rules on the policies and procedures available for appealing eligibility determinations made by MNsure. All comments on these proposed rules must be received no later than 11:59 p.m CDT on Monday, August 12, 2013. The full text of the proposed rules and the request for comments announcement are available on MNsure's website.
Health Care Homes- safety net primary care transformation report available
The Safety Net Primary Care Transformation Grant, funded by the Minnesota Department of Health, provided expert support and facilitation for Safety Net Clinics to facilitate primary care transformation. Halleland Habicht Consulting, LLC was awarded the grant in May of 2011. The final report from this project is now available at http://www.health.state.mn.us/healthreform/homes/background/index.html.
