Minnesota Health Care Homes Patient Experience Survey Webinar
New Information on CG: CHAPS Patient Experience Surveys
Implications on moving from the current CG:CHAPS tool to the new CHAPS 12 month version and PCMH version.

Health Care Homes (HCH) will host a patient experience survey educational webinar. The Minnesota health reform law includes requirements for all physician clinics with sufficient patient volume to collect and report the Clinician and Group Consumer Assessment of Healthcare Providers and Systems (CG-CAHPS) visit specific survey. Another version, the CG-CAHPS 12-month survey, has been recently recommended as a proposed change to the Statewide Quality Reporting and Measurement System. The 12-month survey tool allows users to add supplemental questionnaires, such as the Patient Centered Medical Home supplement which is applicable to HCHs. To provide clarity to the different survey tool options, CG-CAHPS expert Dale Shaller will review the survey tools, and provide an overview of implementation and vendor considerations.

Introduction to Health Care Homes webinars
The Health Care Homes Learning Collaborative is hosting a webinar series entitled, "Introduction to Health Care Homes," which is designed to increase knowledge of innovative practices and standards in primary care, and help equip clinics to begin the transformation process. The webinar series will help clinic teams identify and plan for closing the gaps in care, and will prepare teams to engage in the process of changing culture into patient-centered, team-based coordinated care models.

Statewide Quality Reporting and Measurement System meeting

Health Care Homes webinar on successful approach to asthma care
The Health Care Homes Learning Collaborative is hosting a webinar titled, "Concepts of Co-Management: Reflections from a Primary Care Journey to Improvement Asthma Care for the Pediatric Population," on Monday, July 29 from 12:30 PM – 1:30 PM (Central Daylight Time). In this webinar, Dr. Deborah McWilliams will present on a successful population-based approach, undertaken by the Mayo Clinic – Employee and Community Health program, in caring for 3,000 children with asthma. The success has hinged on the use of RN care coordinators and a greater shift to non-visit communications with patients. Focus has also been placed on the role of the health care home in bridging asthma care across the continuum including subspecialists, schools, and the community. Weekly Update - July 19

MDH announces new health care homes
The Minnesota Department of Health announces 2 newly certified health care homes. There are now 260 certified clinics. Newly certified health care homes are Hennepin County Medical Center Parkside Internal Medical Clinic and Hennepin County Medical Center St. Anthony Village Clinic.

Weekly Update - July 5

U of M researchers expect "welcome mat" effect will drive expansion of Medicaid rolls
U of M researchers say several factors will encourage people who are eligible for Medicaid, but haven't enrolled before, to sign up. The study calls this the "welcome mat" effect. Between the "welcome mat" effect and other factors, Minnesota's Medicaid enrollment could grow by 200,000 people between 2014 and 2016.

ACA employer provision delayed until 2015
The U.S. Department of the Treasury has confirmed that businesses with more than 50 full-time equivalent employees that do not provide employee health insurance coverage will not be penalized in 2014. State officials expect the delay will have minimal impact on Minnesota's health insurance market, according to the Pioneer Press.

Statewide Quality Reporting and Measurement System: Ambulatory Surgical Center Quality Measure Reporting Webinar
On Thursday, July 11, 2013 from 7:30 to 9:00 a.m., MDH and MN Community Measurement (MNCM) will present a webinar for ambulatory surgical center (ASC) staff to review quality measurement requirements, specifications, data submission process and deadlines for quality data reporting. MNCM will send an email with log-in information for the webinar to registered ASCs.
