New SIM Minnesota tool: MAHM Continuum of Accountability Matrix
As part of Minnesota's State Innovation Model (SIM) Grant, MDH and DHS have put together this matrix, which is a tool to illustrate the basic capabilities, relationships, and functions that organizations or partnerships should have in place in order to achieve the long-term vision of the Minnesota Accountable Health Model (MAHM). It is designed to assist the state in identifying criteria and priorities for investment, and to lay out developmental milestones that indicate organizations or partnerships are making progress towards the vision.

Timeframe set for SIM Minnesota e-health RFPs
The Minnesota Department of Health will soon be releasing three Requests for Proposals to support e-health in the Minnesota Accountable Health Model as part of Minnesota's State Innovation Model Grant.

  • Minnesota Accountable Health Model e-Health Grant Program
    The Minnesota Accountable Health Model e-Health Grant Program is designed to support readiness for and participation in the Minnesota Accountable Health Model. The grants will provide funding to community collaboratives to develop a plan for e-health requirements in order to participate in the model or to implement and expand e-health capabilities for participation in the model. It is anticipated that the RFP for the Minnesota Accountable Health Model e-Health Grant program will be released in February, 2014.
  • Minnesota e-Health Roadmaps to Advance the Minnesota Accountable Health Model
    The goal of this Request for Proposals (RFP) is to procure the services of a "neutral third party" to develop and disseminate the Minnesota e-Health Roadmap to Advance the Minnesota Accountable Health Model for the settings of long-term and post-acute care, local public health, social service, and behavioral health. It is anticipated that the RFP for Minnesota e-Health Roadmaps will be released by the end of March, 2014.
  • Privacy, Security and Consent Management for Electronic Health Information Exchange
    The goal of this Request for Proposals is to procure the services for the review of e-health legal issues, analysis and identification of leading practices, technical assistance, and education. It is anticipated that the RFP for Privacy, Security, and Consent Management will be released by the end of March, 2014. Look for announcements at http://mn.gov/sim.

Webinar: Tools and resources to support care of memory-impaired clients, January 28
ACT on Alzheimer's is a volunteer-driven, statewide collaboration preparing Minnesota for the impacts of Alzheimer's disease and related dementias. The collaboration has developed a variety of provider practice tools and resources to support individuals with memory concerns and their care partners. These evidence-based tools incorporate best practices and the expertise of multiple community stakeholders. This webinar will provide an overview of the work of ACT on Alzheimer's and will illustrate the tools primary care staff can use to better serve persons with Alzheimer's disease and related dementias.

Robert Wood Foundation "Bright Spot" Patient Success Story
MDH was contacted by MN Community Measurement to participate in the Minnesota's statewide Aligning Forces for Quality Alliance report. The funder of the alliance, the Robert Wood Johnson Foundation, requested the report include a section describing Minnesota's Health Care Homes (HCH) advancements. This section, called "Bright Spot" highlights a patient success story. All HCH were informed of this unique opportunity to have one of their patient success stories highlighted in the Quality Alliance report. Several HCH submitted stories for consideration. The story of Julia Freeman was chosen.

Statewide Quality Reporting and Measurement System: Clinic Registration and Data Submission Webinar

The Minnesota Department of Health and MN Community Measurement (MNCM) will present an informational webinar to review 2014 clinic and provider registration processes and data submission requirements for the Statewide Quality Reporting and Measurement System (SQRMS) on January 21 (11:30 AM to 1:00 PM) and January 22 (07:30 AM to 09:00 AM). Annual Clinic and Provider Registration Timeline:

Registration must be completed by February 8, 2014. The provider registration guide is posted on the MNCM data portal.

Cycle A Direct Data Submission (DDS) Timeline:

Clinic and Provider Registration and denominator certification must be completed before data can be submitted.

Optimal Diabetes Care and Optimal Vascular Care:

Portal opens: January 13

Portal closes: February 14

Depression Care:

Portal opens: February 3

Portal closes: February 28

Statewide Quality Reporting and Measurement System webinar January 8
On January 8, 2014 from 2:00-3:00 p.m., MDH and Stratis Health will present a webinar to review hospital quality measurement requirements, specifications, data submission processes, and deadlines for 2014 quality reporting. Discussion of the Minnesota Accountable Health Model and public health, January 10
Join Minnesota Public Health Association after the January Policy Forum in a conversation to learn more about the State Innovation Model Grant and Accountable Communities for Health. There will be a panel discussion on the role of public health and community engagement. An overview of the State Innovation Model Grant and Accountable Communities for Health will be presented by Diane Rydrych, director, Division of Health Policy, MDH.
