Weekly Update - August 26, 2010

Statewide Health Improvement Program (SHIP)

Today marks the opening of the Minnesota State Fair, and the topic of MDH’s booth at the fair is SHIP. The exhibit highlights Farm to School initiatives and community and school gardens. It also provides additional background information about SHIP and health reform in Minnesota.

The MDH booth is located in the Education Building, just north of Dan Patch Avenue on Cosgrove Street. The MDH booth is staffed from 9 a.m. to 9 p.m. each day of the fair.

Weekly Update - August 19, 2010

Health care homes

  • The Minnesota Department of Health has certified the first 11 health care homes in the state. The first certified health care homes are in several regions of the state, include both urban and rural clinics and range from single-physician to large systems clinics.

The first 11 certified health care homes in Minnesota are:

  • Christopher J. Wenner, M.D., Cold Springs
  • Mayo Health System-Austin Medical Center
  • Lakewood Health System-Staples Clinic
  • Lakewood Health System-Browerville Clinic
  • Lakewood Health System-Eagle Bend Clinic
  • Lakewood Health System-Pillager Clinic
  • Lakewood Health System-Motley Clinic
  • Park Nicollet-Minneapolis Pediatric Clinic
  • Park Nicollet-St. Louis Park Internal Medicine Clinic
  • Park Nicollet-St. Louis Park Family Medicine Clinic
  • Park Nicollet-St. Louis Park Pediatric Clinic

  • The HealthPartners Research Foundation has received a US Department of Health & Human Services Agency for Healthcare Research and Quality grant to study the transformation of traditional primary care clinics in Minnesota to “health care homes,” also known as “medical homes.”

    Leif Solberg, MD, senior investigator and director for care improvement research at the HealthPartners Research Foundation, received the $596,000 grant to conduct a two-year study, “Evaluating Statewide Transformation of Primary Care to Medical Homes.”

    The Minnesota Departments of Health and Human Services will work with Solberg. With Minnesota Community Measurement and other partners, they will test whether clinics that have transformed their practice by implementing a health care home also see better quality of care of patients with diabetes or heart disease. They will then interview and survey successful clinics to identify key changes most important for transformation.

    The study will also compare more and less transformed clinics in health care costs and utilization and patient and clinician/staff satisfaction.

Weekly Update - August 12, 2010

Statewide quality reporting system

On Monday, August 9, MDH published proposed amendments to Minnesota Rules Chapter 4654 (the Minnesota Statewide Quality Reporting and Measurement System) in the State Register. MDH posted the proposed changes, including changes to the rule’s appendices, at Health Care Quality Measures – Rule Development [expired link]. The publication of the proposed amendments initiated a formal 30-day comment period. Information about how to submit comments is available on MDH’s website. Comments must be submitted by 4:30 p.m. on Wednesday, September 8, 2010. 

A crosswalk of the appendices, comparing the proposed changes published on August 9 to the current rule’s appendices, will be posted at Health Care Quality Measures – Rule Development [expired link] on Friday, August 13.  This document will help stakeholders identify proposed changes to the appendices. 

MDH is required to annually review and publish any proposed amendments to Minnesota Rules Chapter 4654 by August 15 of each year.

Weekly Update - August 5, 2010

Provider peer grouping

MDH will hold monthly conference calls to share information about the provider peer grouping initiative on the second Monday of each month from 7:30-8:30 a.m. The next call-in session is scheduled for Monday, August 9, at 7:30 a.m. The presentation slides for the call will be available Friday, August 6, on the Provider Peer Grouping page.
