The health care homes website has been updated with additional tools in the “education and resources” section. Updates include tools and tips to succeed with the five main health care home certification standards, links to materials from past educational events, and other links and resources.

  • The federal government recently approved the Minnesota Department of Human Services’ (DHS’) proposed payment methodology for health care home services (PDF) provided to fee-for-service enrollees in Minnesota Health Care Programs (MHCP). Approval represents a key step forward in the state’s health care home initiative and ensures DHS’ ability to meet the state’s legislative deadline of making care coordination payments to certified health care homes effective July 1, 2010. The methodology, including a common system of patient complexity tiers, was developed with input from public and private stakeholders to be used by all payers, with rates negotiated privately outside of MHCP. Individual MHCP providers must be certified by the Minnesota Department of Health as health care homes in order to receive the payments.   • The health care homes certification committee will make final recommendations to the Commissioner of Health about health care home certification on July 29.
  • REMINDER: The Institute for Clinical Systems Improvement and Minnesota Departments of Health and Human Services are offering a webinar for clinics, health care organizations, health plans and state systems that are planning to train individuals about the state’s health care home payment methodology process.

    Utilizing a train-the-trainer approach, this webinar will provide strategies and tools to support an internal training program that provides instruction on the payment methodology workflow process and the care coordination tier assignment tool.

  • REMINDER: The MDH health care home team will be hosting informational conference calls and webinars every third Wednesday of the month throughout 2010. Applicants and potential applicants with questions regarding the health care home certification process can call in with questions. There will be a brief update on health care homes implementation and next steps at the beginning of the call, followed by a brief presentation on a certification-related theme. Each month will focus on a different theme.

Provider peer grouping

The July provider peer grouping conference call has been cancelled. The next stakeholder call will take place on Monday, August 9, at 7:30 a.m. 


Statewide quality reporting system

  • REMINDER: MDH seeks public comments on recommendations submitted to the agency related to standardized quality measures. Recommendations have been submitted for both physician clinic and hospital measures and are available at Health Care Quality Measures - Recommendations.

    MDH will accept comments at health.reform@state.mn.us until July 11, 2010.

  • MN Community Measurement’s Measurement and Reporting Committee will meet July 14 on work related to the development of the statewide quality reporting and measurement system.

Weekly Update - July 1, 2010

Health care homes

The Centers for Medicare and Medicaid Services (CMS) recently extended the application deadline for the Medicare Multi-Payer Advanced Primary Care Practice (MAPCP) demonstration. Because of this extension, the Minnesota Department of Health and the Minnesota Department of Human Services have revised the schedule of the stakeholder advisory task force that is offering advice as the state prepares its application.
