Health Care Homes (aka Medical Homes) - Certification & Recertification Requirements and Process
Health care providers must meet a set of standards and criteria in order to be certified as a health care home in Minnesota. There are five basic steps in the application process for certification as a health care home.
Getting started
Submit your Letter of Intent as soon as your organization intends to seek certification. This will allow our team of planners and regional HCH nurse consultants to assist you as needed and provide resources and guidance as you proceed with the certification process.
Please download the printable assessment tool for use internally in your clinic to determine if your clinicians are currently implementing all of the standards and criteria in the rule relating to health care homes. Please note that you should not submit this assessment at this time -- it is simply a tool to help you determine the readiness of your clinician(s)/clinic(s) for certification. Later in the application process you will be prompted to fill in the same assessment tool online. Below are two versions of the printable assessment tool:
- One version for use by clinics for planning purposes

Brochure Template for Providers - MDH is providing a brochure template about health care homes for providers to use with their patients. This brochure template is designed for clinics that are applying to become a certified health care home or that have already been certified as a health care home.

Once a clinic is certified as a Health Care Home, it is the clinics responsibility to maintain accurate provider information using the HCH database. The Post Certification Amend Clinicians Guidance shows you how to add new providers, enter Medicare numbers, correct typos, make name changes, or end eligibility for providers who leave your clinic.
