Health Care Homes (aka Medical Homes) - Certification - Optional Pre-application Activities
Before applying for certification, candidates are encouraged to participate in some optional pre-application activities, which are designed to familiarize them with health care homes and the certification process:
- Read the adopted expedited rule chapter 4764.
- Attend a pre-certification workshop. Please look at Health Care Homes - Events to look for upcoming workshops or slides from past workshops.
- Join a certification informational conference call
- Join the health care home learning collaborative (coming soon)
- Conduct a self-assessment using the health care home certification assessment tool to determine readiness to apply. Please download the printable assessment tool for use internally in your clinic to determine if your clinicians are currently implementing all of the standards and criteria in the rule relating to health care homes. Please note that you should not submit this assessment at this time -- it is simply a tool to help you determine the readiness of your clinician(s)/clinic(s) for certification. Later in the application process you will be prompted to fill in the same assessment tool online. Below are two versions of the printable assessment tool:
