Recertification Step 2 Verify Clinics
An application is completed for each individual clinic and reflects the demographic information unique to the population that clinic serves.
Follow these steps to complete your Online Clinic Application:
- Sign into the Portal.
- From the "Certification Options" dropdown, choose "ReCertify" and then click "Apply for Re-certification".
- In the next screen, click the "Apply for re-certification" link in the "Action" column.
- Under "Step 2 Verify Clinic and clinicians "
- Click on the "Recertify" link in the "Action" column for each clinic.
Please allow enough time to complete the data entry at one sitting as the data for each application is not saved until that clinic application is submitted.
The Application’s demographic data entry in the portal requires the data be in whole numbers (without decimal points). In the areas where percent is used, the % must equal 100% in the total field. You will receive an error message if it does not equal 100%.
