Health Care Homes (aka Medical Homes) - Certification - Review and Notification
MDH will complete its review of all materials and notify the applicant within 90 days of the receipt of a complete application, certification assessment and a site visit.
Receiving materials from the MDH review
- Applicants receive a report identifying which standards are met and not met
- Applicants who do not meet 100 percent of the standards may take one of the following steps:
- Request a variance
- Request support from the learning collaborative/MDH for technical assistance
- Develop a plan for remedy and reapply for those areas not met
- File an appeal
- Request a variance
- Applicants who do meet 100 percent of the standards will receive a certification notice and may begin implementation of the health care home
