Health Care Homes (aka Medical Homes) - Certification - Site Visit
The site visit is a time for MDH to verify that the applicant meets the standards and criteria for health care homes and for the applicant to share information about the health care home.
Arranging a site visit
- Work with MDH to schedule a mutually convenient date for site visit
- Designate staff to lead, prepare for and schedule clinicians/staff for the site visit
- MDH will notify applicant within 90 days of completion of site visit of applicant’s status and next steps
