Health Care Homes (aka Medical Homes) - Learning Collaborative Advisory Committee
Purpose
The primary purpose of this group is to provide guidance and technical expertise to the HCH Learning Collaborative. This committee will work closely with the HCH Staff and Learning Day Planning committees to plan the activities, evaluation and curriculum for the Learning Collaborative.
The primary activities of the committee will include:
- Direction and advice in development of the Learning Collaborative.
- Ongoing support, participation, and advice in the ongoing implementation of learning events.
- Participates and leads other workgroup initiatives to implement new strategies identified by the committee.
- Reviews and advises on policy, evaluation materials, and results, and does problem solving as needed.
