Learning Days Event

The annual Learning Days conference brings together more than 300 health care providers, public health professionals and community partners to exchange information, advance health transformation, and build relationships.

Health Care Homes seeks proposals for concurrent breakout sessions at its daylong learning event on April 10, 2019.

The proposal deadline is November 9, 2018.

Breakout sessions are 60 minutes in length. Proposals may include individual or group presentations (up to four presenters per session). Proposals should align with conference objectives of enhancing knowledge and skills to:

  1. Rethink assumptions about creating health and health equity for all Minnesotans
  2. Redesign care delivery to enhance community health and restore joy in practice
  3. Retool for the transition to value-based payment

We are looking for proposals that reflect best practice for adult learners and:

  • Feature case studies, examples and stories
  • Include patients and consumers
  • Involve interactive activities
  • Include practical take home tools
  • Are free of commercial bias

Learning Topics

Proposals requested for beginning and advanced topics, as follows:

  • Whole Person Care: identifying and addressing social determinants of health and health literacy, whole-person care integration (primary, dental, behavioral, specialty, community providers, etc.)
  • Team Based Care: care team roles and responsibilities, patient engagement and activation, shared decision-making, practicing at top of license, joy in practice
  • Community Partnerships: improving community health through partnerships, using community health assessment data, health information exchange
  • Quality Improvement: quality improvement tools, registries and data analytics, work flows, caseload tools, quadruple aim measurement and continuous improvement
  • Health Equity: health equity assessment tools; addressing health equity within your organization; creating culturally friendly organizations
  • Value-Based Care: understanding total cost of care; preparing for MACRA/MIPS; reimbursement options, grants and alternative funding sources

Download the Proposal Submission Form (DOCX), complete and follow instructions for submission. The Learning Days planning team will review and select proposals to present a balanced agenda of learning experiences for stakeholders at all stages of transformation.

Additional Considerations

  • All speakers must register for the conference. There is one free speaker registration per session; additional speakers are required to pay.
  • Submit PowerPoint presentations by February 15, 2019. PowerPoints are posted to the Health Care Homes website and must comply with the Americans with Disabilities Act.
  • Speakers will assume responsibility for any printed materials they wish to distribute.
