Health Care Homes (aka Medical Homes) - Learning Collaborative - Phase I
Phase I: Preparation for HCH Certification
Phase I of the Health Care Home Statewide Learning Collaborative is a voluntary, collaborative learning series for clinics and teams that are preparing for certification or have an interest in becoming certified.
This collaborative will consist of three months of virtual learning through two 90-minute webinars and two one-hour conference calls. Additionally, there will be post-learning activities designed to apply the learnings and engage the broader team.
Participants may register for one of the following three series (all three series cover the same content):
- Group One: May 25 – August 5, 2011
- Group Two: September 6 – November 4, 2011
- Group Three: November 7 – January 12, 2012
