Health Care Homes (aka Medical Homes) - Learning Collaborative - Phase III
Phase III: Networking and Recertification
Phase III of the statewide learning collaborative is required for all certified health care home clinics and/or clinicians. Completion of Phase II is required in order to participate in Phase III. This phase is to provide participants an opportunity for networking and ongoing improvement and maintenance through virtual discussions via conference calls over a three-month period.
