Phase II: Transforming and Certified Clinics
Phase II of the statewide learning collaborative is required by law for all health care home certified clinics and/or clinicians that have begun implementation of the health care home components.
This phase of the collaborative is six months long and consists of two four-hour face-to-face sessions, two 90-minute webinars and two one-hour conference calls. The collaborative will consist of different cohorts of teams and schedules with locations at various regional areas throughout Minnesota. Additionally, there will be post-learning activities designed to apply the learnings and engage the broader team.
