Health Care Homes (aka Medical Homes) - Consumer/Family Advisory Council

This council provides consumer/family perspectives and expertise to assist in the development, implementation and evaluation of patient/family-centered health care homes in Minnesota. 

The council began meeting in November 2008. Members have served on each of the health care home work groups and committees that have been working to implement the health care home provisions in Minnesota's 2008 health reform law. These work groups have covered subjects including standards and certification; verification; payment methodology; and outcomes measurement.

Structure

The Minnesota Department of Health (MDH) and the Minnesota Department of Human Services (DHS) are seeking additional council members. Consumers, family members or health advocates who represent other consumers and families can participate. Members must have diverse experience utilizing health care services and be willing to share their experience and expertise to enhance health care homes - and bring information back to the organization or association they represent. 

The goal for the consumer/family council is to include:

  • Diverse representation of chronic health and development conditions (including physical and mental health)
  • Diverse cultural, linguistic and socioeconomic composition
  • Diverse ages of patients being represented

MDH and DHS seek members who meet these criteria. Members may include representatives of major advocacy organizations. The consumer/family advisory council meets independently, facilitated by MDH/DHS with the assistance of a contracted facilitator.
