Purpose
- To ensure primary care practices receive the education and resources needed for successful implementation of the health care home (HCH) model in Minnesota.
- To serve as a vehicle for communication on HCH to members/employees of the participating organizations.
Principles
- The committee should provide practical materials that are ready to use, with special emphasis on those groups that have been successful in implementing the medical home model and may have insight into what tools and resources would have been helpful.
- The committee should coordinate with the Health Care Home Consumer/Family Council related to patient/consumer materials.
- Members should be supporters of health care home concepts, willing to make them work in clinic settings throughout Minnesota, have credible resources and materials to bring to the table, and be willing to promote education/training events to their constituencies.
Role
To provide advice and recommendations, in collaboration with MDH staff, on:
- Educational resources for clinics or clinicians preparing to apply for HCH certification, based on the health care home standards and rules.
- Process and criteria for reviewing and recommending material.
- Guidance for the development and implementation of 2009-2010 HCH educational events.
- Criteria for the selection of teams to receive scholarships to attend the educational events.
- Technical advisers to review specific tools and provide feedback and guidance between meetings.
- Criteria for the selection of clinicians as HCH mentors and speakers bureau.
- Criteria for the selection of clinics to receive mini-grants to start patient and community engagement and technology mini-grants to support HCH implementation.
Structure
- Committee members (15-16) represent broad perspectives and experiences with the implementation of medical home concepts and tools.
- From November 2010 through February 2011, meetings will be monthly.
- Committee members are asked to participate in the active review of materials in between meetings.
