Health Care Home Capacity Assessment Project
The Minnesota Academy of Pediatrics Foundation partnered with the Minnesota Academy of Family Physicians, the Minnesota Chapter of the American College of Physicians and Stratis Health on this project.
- Read the final report evaluating collaborative learning methods and models that incorporate quality improvement approaches that could be implemented statewide for initial and ongoing clinician certification as a health care home provider. This is the first step in developing the new learning collaborative for health care homes in Minnesota.
