Health Care Homes Community Meeting - December 12, 2008
About 120 people attended the Health Care Homes Community Meeting on Friday, December 12. Minnesota Commissioner of Health Dr. Sanne Magnan and key health care homes staff from the departments of health and human services spoke about the concept of health care homes, health reform legislation and the process for forming health care homes in Minnesota.
