Health Care Homes (aka Medical Homes) - Mini-grant Awards

Mini-grant awards - February 2010

Twenty mini-grant applications from across the state were selected to be funded as part of a Minnesota health care home development project focused on patient/family-centered care. Over 65 proposals were received from clinics or systems throughout Minnesota. The proposals focused on ways to increase patient/family participation in clinics and emerging health care homes. Examples of funded projects include:

  • Establishing an active Patient/Family Advisory Committee and patient mentoring programs
  • Compensating patient/family representatives who serve on a Health Care Home and Patient Advisory Council
  • Engaging patients in clinic's quality improvement projects
  • Structuring and implementing community focus groups, including Hmong immigrants
  • With patient/parent input, developing a record-keeping system that provides convenient access to care plans and all appointments/records.
  • Providing mileage reimbursement for Community Quality Improvement (QI) representatives to the Clinic QI teams
