Health Care Homes (aka Medical Homes) - Mini-grant Awards
Mini-grant awards - February 2010
Twenty mini-grant applications from across the state were selected to be funded as part of a Minnesota health care home development project focused on patient/family-centered care. Examples of funded projects include:
- Establishing an active Patient/Family Advisory Committee and patient mentoring programs
- Compensating patient/family representatives who serve on a Health Care Home and Patient Advisory Council
- Engaging patients in clinic's quality improvement projects
- Structuring and implementing community focus groups, including Hmong immigrants
- With patient/parent input, developing a record-keeping system that provides convenient access to care plans and all appointments/records.
