Data Submission

As a requirement of the certification process, Health Care Homes are required to submit case-level data for evaluation and benchmarking purposes. For more information about this requirement, please refer to the document:

Minnesota Community Measurement (MNCM) and MDH collaborate to inform certified Health Care Homes about the requirements for the data submission process.

Measures have been updated for 2018. These measures are:

  1. Depression Care: Remission at Six Months
  2. Optimal Diabetes Care
  3. Optimal Vascular Care
  4. Colorectal Cancer Screening
  5. Optimal Asthma Control / Asthma Education and Self-Management
  6. Pediatric Preventive Care: Adolescent Mental Health and/or Depression Screening

A complete list of physician clinic and hospital quality measures, Quality Rule Appendices (which includes measures, data elements, and specification information), measure specifications, and variance information for the 2018 reporting year are available on the MDH SQRMS website.
