Health Care Homes (aka Medical Homes) - Performance Measurement and Evaluation
The Health Care Homes Performance Measurement Work Group is charged with developing performance measurement implementation strategies and making recommendations on how health care homes performance measurement can work within the broader statewide reporting structure. This work group receives reports from the health care homes team and responds to the progress of performance measurement, ongoing identification of outcomes and evaluation of the progress of health care homes measurement over time.
This work group began meeting in 2009 and continues to meet regularly.

MN Community Measurement and MDH held a webinar in October 2012 to present information about benchmarking to certified health care homes.
