Health Care Homes (aka Medical Homes) - Outcomes Measurement Development
The Health Care Homes Outcomes Measurement Work Group is charged with developing outcomes measurement implementation strategies and making recommendations on how health care homes outcomes measurement can work within the broader statewide reporting structure. This work group receives reports from the health care homes team and responds to the progress of outcomes measurement, ongoing identification of outcomes and evaluation of the progress of health care homes measurement over time.
This work group began meeting in 2009 and continues to meet regularly.
Certified Health Care Homes Data Submission
MN Community Measurement and MDH held a webinar in April to inform certified health care homes about the requirements for their first data submission.
