Take the Health Care Homes Standards Development Survey
Thank you for taking the time to offer your feedback about standards for the certification of health care homes in Minnesota. The formation of health care homes is part of the health reform legislation that was signed into law in May 2008. As part of our process for developing standards for health care homes, we are seeking broad input from a wide range of stakeholders. Our work groups have been meeting and are in the process of drafting standards, and we'd like to get public response as we continue this important effort.
Please rank each of the standards using the scale provided, Strongly Disagree, Disagree, Agree, Strongly Agree or No Opinion, for each of the three questions:
- This standard will make a difference in how I / my patients receive care
- This standard is achievable to implement
- This standard is important for achieving our outcomes for patient experience, health and cost.
The deadline for your response is January 7, 2009. Your feedback from this survey will be integrated with the work groups' discussions. The work groups will review and hone the standards in additional meetings in mid-January before providing recommendations to the Commissioners of Health and Human Services.
