MNsure Provider Networks
MNsure, Minnesota's one-stop health insurance marketplace, lets people find and compare health plans and choose the coverage that meets their needs. The health plans offered by MNsure are required to meet multiple certification standards. MDH is responsible for certifying provider networks including physicians, specialists, clinics, hospitals, and other providers.
Provider Networks available on MNsure in 2018
You should contact a health insurer or HMO directly with questions about individual networks. Please visit the web sites listed below to get the most up to date provider network information from each health insurer or HMO. Networks are available for health plans sold to individuals and families, and small employees (50 or fewer employees). An individual health plan can be purchased by a consumer and can include family members. Small employer group plans are approved for 2018, and are available by contacting an insurance broker.
Health insurers offering a health care provider network for individuals and families, and small employers need to meet certain state standards for geographic access. In some situations they may be unable to meet these standards and are required to submit a Provider Network Waiver request.
