Notice of Availability of Contract for Implementation of a Statewide Health Care Homes Learning Collaborative

The Minnesota Department of Health is requesting proposals for the purpose of developing and implementing a statewide health care homes learning collaborative that provides an opportunity for health care homes and state agencies to exchange information and enhance understanding related to quality improvement and best practices on the implementation of health care home standards and rules.

Work is proposed to start after June 1, 2010.

A request for proposals will be available by mail from this office through April 9, 2010. A written request (by direct mail or fax) is required to receive the request for proposals. After April 9, 2010, the request for proposals must be picked up in person.

Proposals submitted in response to the request for proposals in this advertisement must be received at the address above no later than 4 p.m. Central Daylight Time on Monday, April 26, 2010, or have a legible postmark from the United States Postal Service or a legible pick-up or drop-off time from a private carrier with a date and time before 4 p.m. Central Daylight Time, April 26, 2010. Late proposals will not be considered. Faxed or e-mailed proposals will not be considered.

This request does not obligate the State to complete the work contemplated in this notice. The State reserves the right to cancel this solicitation. All expenses incurred in responding to this notice are solely the responsibility of the responder.
