  1. As stated in the Grant Amendment for October – December 2011, grantees are required to submit a report for the grant amendment period October 1, 2011 through December 31, 2011. Below, you will find a link to the 2011 Grant Amendment Quarterly Report form. Please note that reports should be completed and submitted as a single Word document to Diane Lauren at diane.lauren@state.mn.us.  Reports are due by January 31, 2012.

    Information collected in the 2011 Grant Amendment Quarterly Report will be used to (1) track additional statewide progress toward expected outcomes and (2) report to the Minnesota State Legislature and other key stakeholders. Metro Adult Health Survey

The Metro Adult Health Survey is a regional, coordinated public health effort to provide standardization and comparability of health data for the metro region of Minnesota.  Results from the survey provide county-specific baseline data on health behaviors (physical activity, healthy eating and tobacco use and exposure) and overall health status of adults residents in six metro area counties – Anoka, Carver, Dakota, Ramsey, Scott and Washington.  The survey was made possible through funding from the Statewide Health Improvement Program (SHIP).
