Bed Count Survey
You should receive an email when bed count is needed. At that time, choose the correct survey for your region, based on the county where your facility is located.
- Central Region Skilled Nursing Home Bed County Survey
Benton, Cass, Chisago, Crow Wing, Isanti, Kanabec, Mille Lacs, Morrison, Pine, Sherburne, Stearns, Todd, Wadena and Wright counties.
- Metro Region Skilled Nursing Home Bed Count Survey
Anoka, Carver, Dakota, Hennepin, Ramsey, Scott, and Washington counties.
- Northeast Region Skilled Nursing Home Bed County Survey
Aitkin, Carlton, Cook, Itasca, Koochiching, Lake, and St. Louis counties.
- Northwest Region Skilled Nursing Home Bed Count Survey
Becker, Beltrami, Clearwater, Hubbard, Kittson, Lake of the Woods, Mahnomen, Marshall, Norman, Pennington, Polk, Red Lake, and Roseau counties.
- Southcentral Region Skilled Nursing Home Bed Count Survey
Blue Earth, Brown, Fairbault, LeSueur, Martin, McLeod, Meeker, Nicollet, Sibley, Waseca, and Watonwan counties.
- Southeast Region Skilled Nursing Home Bed Count Survey
