Acknowledging EHDI Support Provided by MDH/OMMH
Grantees are asked to acknowledge support from the Minnesota Department of Health Office of Minority and Multicultural Health by using the agency and division’s logos and credit line in all printed materials, news releases, newsletters, advertisements, postcards, posters, programs, catalogs, invitations, films, videotapes, electronic transmissions and Internet sites.
The credit line should contain the following language:

Both the MDH logo AND the OMMH logo should be included. It should NEVER be just the OMMH logo as a stand-alone. The logos are available for download below.
Grantees are also asked to acknowledge MDH/OMMH support even when written credit is not applicable, by providing oral credit before each event or performance.

Photos submitted to OMMH for their printed materials or website need to have signed photo releases from individuals in the photo. Have each person in the photo sign and date the Photo, Video and Audio Release Form (PDF: 17KB/1 page). All names can be included on a single form.
