Eliminating Health Disparities Initiative Goal: Develop materials to encourage women to receive screenings for breast and cervical cancer and to collaborate with Bois Forte Health Services community health program to provide case management for the women using health services and provide transportation as necessary.

Eliminating Health Disparities Initiative Goal: Reduce infant mortality and encourage healthy youth development by providing education-appropriate referral services.

Eliminating Health Disparities Initiative Goal: Organized physical activities and fitness education for community members under the guidance of a fitness trainer. Exercise is a high priority health improvement activity. All Prairie Island members are encouraged to participate in these activities.
