Reminder about Risk Factor 425X
Risk Factor 425X – Child Transitioning to New Age Category should only be used if you are unable to create a default food package for a child who is changing from one age category to another and isn’t currently assigned a risk factor that applies to the new age category.
This risk factor was created when HuBERT was rolled-out because when a child transitions from one age category to another (at 1, 2 and 3 years of age), they don’t always have a risk factor that carries forward to the new age category.
When this occurs and staff try to create an age-appropriate food package, they aren't able to use the "Use Default" function. Since creating a food package from "scratch" is time-consuming, and can be difficult if staff aren't familiar with the food package, Risk Factor 425X was created as a work-around to allow staff to create default food packages when there aren't any age-appropriate risk factors. It should never be assigned as the only risk factor at certification. It should only be used when other risk factors have been assigned and you are unable to create a default food package.

Complete Health Information before Assigning Risk Factors
This is just a reminder that risk factors are based on both WIC Type and Breastfeeding Amount. In order to assign risk factors, the Health Information should be completed first. Otherwise, there won’t be any risk factors assigned by the system or available to assign in the Assign Risk Factors screen.

How Risk Factors Display in the Risk Factors Tab when Assigned from the Nutrition Assessment Tab
Risk factors, when assigned while in the Nutrition Assessment Tab (NAT) during a certification, display under the dates of that certification on the Risk Factors tab.
If another NAT is completed during the certification period, either via the Participant Folder or the Mid-certification Assessment, HuBERT does not go back and retrieve the risk factors assigned when the previous NAT was completed; it creates a new record of risk factors associated with the new NAT.
In the Risk Factors tab, if a pseudo-cert is created upon completion of a new NAT, only those risk factors assigned during the new NAT will display under the dates associated with the pseudo-cert while the risk factors assigned during the initial NAT will continue to display under the dates of the certification.
