Adding New Medical Conditions - Medical Cannabis Program
You may petition the Department of Health to add a qualifying medical condition. The department accepts petitions from June 1 to July 31 each year. Petitions received outside this window will not be considered. Please refer to the Adding New Medical Condition Petition for the 2017 submission.
You may now begin the process of completing a petition to add a new medical condition for the 2017 window. Please refer to the Petition to Add New Condition 2017 (PDF) form.
