This website is part of Healthy People 2020—the national effort to set measurable goals and objectives for health improvement over a ten year time frame. This page provides an overview of the social determinants of health and identifies health objectives related to these determinants. In the overview there is a link to national data on the health objectives that is broken down by race, sex, age, and other variables. There are separate tabs that list the health objectives, and links to resources about effective strategies that address social determinants of health. Across the top of the page there are links to other Healthy People 2020 resources, including, in the Tools and Resources section, a link to county level data resources.
