Asthma Friendly Schools Mini-Grant Program
The Minnesota Asthma Friendly Schools Mini-Grant Program was developed by the Minnesota Asthma Program to support schools in becoming more asthma friendly.
Grants of up to $1,500 are available to implement a variety of school-based projects. Minnesota K-12 schools are invited to apply. Projects may be implemented in one or more schools or district-wide.
Grant application process is now CLOSED for the 2022 school year.

How to apply
- Review ALL criteria
- Review all content areas on the mini-grants webpages including links associated with the project you choose.
- Choose a project – consider what key partners are needed to support your project and make sure they are part of the application submission process.
- Review both the OUTCOMES REPORT and DATA REQUEST forms.
- Create your own (2-3) measureable goals for your project.
- Complete and submit online application.
Things to know
- An MDH Asthma staff member will set up a time to talk with you about your application.
- Applicants who have completed their first project may apply for additional funding to work on new projects.
- Submit your application as soon as possible to allow time for MDH to process your application, provide any technical assistance you may need and give you sufficient time to complete your project.

Projects are organized into seven topic areas. You can apply to complete a single project or multiple projects. Each project does not have to be from the same topic area. All required documents are provided. Be sure to review the Outcomes Report Form as you design your project so that you are familiar with the type of information you will need to collect and report. The project descriptions include links to resources that may be useful in completing your projects.

- Health Office Asthma Data
Complete an asthma data project involving the school health office's data management system.
- Environmental Policy
Address school policies that support asthma-friendly school environments.
- Partnerships and Building Awareness
School health staff will partner with at least two health care professionals serving their community (such as local public health, clinics, local pharmacies, non-profit organizations, etc.) to organize and hold an asthma educational event.
- Asthma Self-Management Education
Provide asthma self-management education in a group or individual setting or a combination of both.
- Linking Schools and Health Care Providers
Enhance communication and collaboration between schools, clinics, hospitals, and public health.
- Health Office Staff and School Personnel Training
Enhance the knowledge and skills of health office staff and other school personnel related to asthma, asthma self-management and best practices for asthma management in schools.
- Build Your Own Asthma Project
School personnel and their partners will 'Build Your Own' school and community focused asthma program.
Before beginning your application, review the Funding Criteria to ensure your project meets the grant requirements.
Applications must meet all project specifications as indicated under the selected project choice.
- Funding may not be used to pay for the development of individual health plans or direct nursing care services that are normally provided. The mini-grant program is not designed to support basic nursing services expected as part of the daily health office routine.
- Funding may be used to pay for staff time including, for example, substitute school staff to allow others to attend a training or to develop project deliverables.
- Mini-grants will not cover work completed prior to approval of your application.
- Funds may not be used to purchase equipment (e.g., peak flow meters, holding chambers) for use by individual students.
- Funds may be used to purchase demonstration equipment for the school health office and to support asthma self-management education. *Note that applications for ‘Providing Asthma Self-Management Education’ projects are expected to budget for holding chambers for all students involved in that project.
- Funds may not be used to purchase food, beverages, incentives or prizes.
Applications must originate and be submitted by the school or district implementing the project.
- Schools/districts are encouraged to partner with outside organizations.
- Grant applications must be coordinated with the school health office.
- The fiduciary agent must be the school or school district but they may subcontract services to build sustainable programming.
- Projects must be implemented in elementary, middle or high schools in Minnesota and can take place in one or more schools or districtwide.
- Timeline and project activities should not reference application tasks accomplished before application approvals are completed.
All applicants must:
- Provide contact information and a signature from your school or district administrator so that we know your school or district health office supports your application.
- Provide contact information for any outside partners or organizations with whom you plan to work and a short explanation of the nature of their contribution to the project.
- Provide details that show how asthma affects students in your school or district and how this grant will help your school and students improve asthma control.
- Include a brief description of how you plan to partner with others to build, implement, and sustain your asthma programming once mini-grant funding is gone.
- Develop realistic measurable goals for each project. (See sample Goals (WORD) document below).

Each grant recipient will be asked to complete an Outcomes Report for each project selected. Review the Outcomes Report while planning your project to ensure the questions can be answered when you complete your project.
We ask that all documents, PowerPoint Presentations, and materials you create to support your project be shared with the MDH Asthma Program. These materials, with your permission, may be shared with other mini-grant applicants as examples for their projects.
Confirmation and Notification
MDH will send you an email confirmation that we have received your online application. If you have not received this confirmation within 2 weeks of your submission, please email firstname.lastname@example.org or 651-201-5909
MDH will contact you to set up a time to discuss your online application. Final payment is sent only after MDH receives all invoices, receipts, completed Outcomes Report and Data Requests for each project(s).
