COVID-19 Notification Letter Templates for Schools, Child Care, and Youth Programs

The Minnesota Department of Health (MDH) provides templates of letters that schools, child care, and youth programs can use to notify families and staff about COVID-19 infections and close contacts.

Privacy note: To protect the privacy of the person who has COVID-19, notifications should contain no information that may potentially identify them. When using these templates, be sure to customize them by including important information about the infection, such as dates and where contact happened. However, DO NOT edit the templates in any other ways, as they have gone through an extensive review process at MDH.

Close contact notification

Use the appropriate letter below when a student, a child in child care, or a staff member is identified as a close contact of someone who tested positive for COVID-19 and must stay home. The letters explain that the close contact must stay home and may not go to school, work, child care, or any other activities during their quarantine period of 14 days, 10 days, or 7 days depending upon the option.

Quarantine Guidance for COVID-19 (PDF)
This document can be sent with the notification letters to people who are identified as close contacts of people who have COVID-19. The guidance outlines the 14 day, 10 day, and 7 day quarantine options and the requirements for each option.

General notification

Use the appropriate letter below to inform families or staff that a person in the school/child care/youth program community tested positive for COVID-19 and to tell them their child or they have not been identified as a close contact of that person.
