2020 Innovator Awards First-Place Winning Team to Hennepin Healthcare
Hennepin Health, a safety-net accountable care organization (ACO) serving Medicaid patients in Minneapolis, was named as a first-place winner in Healthcare Innovation’s 2020 Innovator Awards Program for their leading edge work in addressing social determinants of health (SOD). Their work created change through a comprehensive, multi-layered initiative designed to properly address patients’ social needs that are leading contributors to high healthcare utilization, and then appropriately intervene with care management for certain underserved members. An analytics tool was created to comb through the electronic health record (EHR) for common risk factors indicating housing insecurity, including patient address corresponding with a homeless shelter, prior visits to Hennepin County Health Care for the Homeless, medical diagnoses of homelessness, and previous social assessments from across the health system.
