Submit a Record Request to MIIC
Requests will be processed in the order they are received. Due to a high volume of requests, it may take up to 21 business days to process your request.
A parent or legal guardian can request records for children under the age of 18 years. People over the age of 18 years old must request their own record. You cannot request a record for your spouse/partner unless you are their legal guardian.
If you are looking for immunizations before 2002 or have lived in another state besides Minnesota, that information may not be available. Although most health care facilities submit immunization information to MIIC, not all facilities do.
