County Office Surcharge Reporting
County vital records offices must remit vital records surcharges they collect to Minnesota Management and Budget (MMB). We, the Office of Vital Records, want counties to remit surcharges to MMB monthly. Monthly remittance to MMB and monthly reporting to OVR will help OVR track "fee revenue" (surcharges) so that we do not spend more than we collect.
All vital records offices charge a $9 “base” fee for the administrative review and processing of a request. The law adds nonrefundable surcharges to the base fee. Below is the list of nonrefundable surcharges.
- $3 for each certified birth or stillbirth record and for each certified statement of no birth record found
- $10 for each certified birth record
- $4 for each certified birth or death record, and for each certified statement of no record found
- $4 for each noncertified birth or noncertified statement of no birth record found

File your monthly report using the Vital Records Surcharge Reporting form.
Report this information no later than the 10th of the month following the issuance of certificates.
