Applications

• Asbestos Certification - Apply Online for new applicants. 
  To apply for asbestos worker, site supervisor, inspector, project designer, or management planner certification.
• Asbestos Contractor License - Apply Online for new applicants. 
  To apply for an asbestos contractor license.
• Asbestos Training Course Permit - Apply Online
  To apply for a training course permit at least 60 calendar days in advance of holding the course.

Notifications

• Asbestos Training Course Notification - Apply Online
  Training course providers must notify MDH 14 calendar days before the course begins with this form.
  • Training Course Notice - Student Roster
    Fill out the name and diploma number of each student that attended and passed your training course.
• Notification of Asbestos-related Work - Apply Online
  To notify MDH of regulated asbestos work at least five calendar days before the project begins for initial notifications.
  • Asbestos Work Notice Amendments
    Notify MDH about amendments or cancellations to asbestos-related work notifications.
