SHIP Monitoring Partner Sites Project Guidance
Entering new SHIP partner sites
A partner site is defined as a physical location where SHIP strategies are implemented. Partner sites include organizations such as businesses, community-based organizations, local government offices, faith and cultural organizations, and local jurisdictions such as cities and counties.
To enter new partner sites into REDCap, go to SHIP Partner Site Registration Form. Information entered in this form includes the name and address of partner sites, strategies, and activities.
While the Partner Site Registration form is always open, your partner sites will not immediately show up in REDCap. See the SHIP Grantee Due Dates (PDF) document for dates when partner sites are uploaded.
Updating strategies, activities and partner site status
All information about strategies, activities, reach and PSE is updated in the “2020 SHIP Monitoring Partner Site” project in REDCap. Local SHIP staff can enter information into this project between Feb. 10, 2020, and Aug. 28, 2020.

Collaborative Partners Project Guidance
Collaborative partners can be multi-organizational collaborations, coalitions, networks, partnerships, or workgroups for shared learning, networking, and/or systems coordination that SHIP staff participate in.
Collaborative partners do not include:
- Community Leadership Teams
- SHIP workgroups
- Workplace collaboratives
- District or school wellness committees
All information about collaborative partners is updated in the “2020 Collaborative Partners” project in REDCap. Local SHIP staff can enter information into this project between Nov. 1, 2019, and Oct. 31, 2020.

Community Leadership Team Project Guidance
Local SHIP staff are required to convene Community Leadership Teams (CLT) to inform the development of SHIP activities in their region. The Minnesota Department of Health requires reporting on CLT members and meetings through REDCap as part of regular SHIP grant monitoring.
SHIP requires the completion of two projects in REDCap following this data entry schedule:
- CLT Member List (Up-to-date list of current CLT members, their sector and role) DUE: May 31, 2020, and Nov. 30, 2020
- CLT Meeting Log (List of meetings held and number of attendees) DUE: Nov. 30, 2020

Local Health Assessment and Data Collection Guidance
All SHIP grantees are required to collect core behavioral questions as part of their local adult community health assessment surveys to guide local SHIP activities. The second round of data collect needs to be completed by Oct. 31, 2020. The SHIP Local Health Assessment Data Collection Requirements (PDF) includes information on data collection methods, timeframe, and required questions.
