May 31, 2021 Renewal Announcement
The renewal notice packet for practitioners whose current license will be expiring on 05/31/2021 is scheduled to be mailed out on 03/31/2021.
If you changed your address since your last renewal and you did not notify MDH, you may not get your renewal packet. Please follow the address change instructions if you need to update your contact information with MDH.
If you do not receive your renewal notice packet by 04/12/2021, please send an email to Health.SLPA@state.mn.us with the subject “Request for Renewal Notice.”
Notice - Do not complete the background study process with DHS
If you are an applicant for licensure or certification or are renewing your license or certification with the Minnesota Department of Health, do not start the background study/criminal background study process with the Department of Human Services (DHS) until the end of the Minnesota peacetime state of emergency. We will contact practitioners when the complete background study/criminal background study process has resumed.

The renewal notice packet for practitioners whose current license will be expiring on 01/31/2021 are scheduled to be mailed out on 11/30/2020.
If you changed your address since your last renewal, and you did not notify MDH, you may not get your renewal packet. If you do not receive your renewal notice packet by December 12, 2020, please send an email to Health.SLPA@state.mn.us with the subject “Request for renewal notice.”
Please note that Speech Language Pathologists and Audiologists whose licenses are scheduled to expire on 01/31/2021 were required to complete a background study during this renewal, as required by Minnesota Statutes, § 144.0572, subd. 1. Due to the COVID-19 peacetime emergency declared by the Governor, and referenced in the June 5, 2020 Health Occupations Program Modification Waiver, this process will be delayed until after the COVID-19 peacetime emergency is rescinded. We will notify you by email when you can start the fingerprint criminal background check process, which is conducted by the Minnesota Department of Human Services, through NETStudy 2.0.
We will process your renewal after we receive your online renewal and your renewal payment. We will not require the background check until after the Minnesota peacetime state of emergency has ended. Please note that your Continuing Education (CE) for this reporting period should be taken from 02/28/2019 to 01/31/2021. Practitioners are required to report no more than 30 hours during their CE reporting period. You may submit your CE report during or after you renew your license.
Important Information Regarding the 10/31/2020 License Renewal
The expiration date for practitioners whose licenses were scheduled to expire on 5/31/2020 was extended to 10/31/2020. We began processing these renewals the week of September 28 with a goal of mailing all license renewals before November 1.
To get started, reference the letter we sent to you on March 18, 2020, which contains your user ID and password. You will need to log in and complete the online renewal process. During this online process you will be able to upload your continuing education (CE) reporting form. We recommend that you upload your CE reporting form to expedite the renewal process. If you are unable to upload it, please email it to Health.SLPA@state.mn.us.
Please keep a copy of the email that you send for your records. Once you have completed the online renewal application you will be given the option to pay the renewal fee. We recommend that you make your payment online to expedite the renewal process. If you have already completed the online process and paid the renewal fee you will not need to submit it again.
We have received notification from the Department of Human Services that they are now processing background studies including the fingerprinting process. If you have not already applied and paid the fee for the background study to DHS please do so now. You can access the background study instructions at Applicant Background Study (PDF).
If you have already completed the background study application and paid the Department of Human Services for the background study you should receive an email from DHS in approximately five to ten business days. The email will contain information on where to go to get the fingerprint process started.
If you have already completed the background study for your Minnesota Department of Health license/certification and were deemed clear by DHS you do not need to submit another background study.
We require a surcharge fee for our office to process the background check. The surcharge fee for audiologist is $13 and the fee for speech language pathologist is $10.50. If you have already mailed a check for the surcharge fee to our office, you do not have to mail another one. If you have not yet paid the fee, please mail a check or money order payable to the Minnesota Department of Health to:
